Question

I had a biopsy that shows portal inflammation grade 1 on ...

I had a biopsy that shows portal inflammation grade 1 on scale 0-4, portal fibrosis grade 1 on scale 0-4, hepatocellular iron 2-3 on scale 1-4, fatty change grade 1 on scale 0-4. Heavy drinker for 20 years. quit but after the biopsy i took clonipine and trazadone meds for 3 months daily from a doctor. I have all the symptoms of cirrhosis now after these meds. Think the meds pushed my liver over the limits. My symptoms are unbearable. Went to my primary doctor he says to have the symptoms your liver would have to be shut down 90%. He does blood work liver function test and they come back within normal limits. But those test do come back normal with cirrhosis. Those test mostly show active damage going on. Now im kinda lost i know my body and every symptom i get i look it up and its a symptom of liver disease. They say the liver is a very complicated organ if you hurt it. qustions how does my biopsy report look, is my liver is bad shape. with the damage i have how much more has these pills done. Could i be right that yes i do have cirrhosis. Not sure what any doctor can do at this point.

I think that this has been discussed before too, however since you have asked a fresh question, here is what I feel,
The symptoms of cirrhosis indicate a very significantly deranged liver. These would require a definitely abnormal liver function tests.
The like biopsy report is not suggestive of advanced liver disease/ cirrhosis, more like fatty liver damage (due to possibly alcohol use) and inflammatory changes (? drug related)
These are just educated guesses but based on the most likley scenario.
I believe that these reports are overall unlikley to lead to that you should have classical symptoms of cirrhosis,

Thanks, but thats my problem. This biopsy should not make me have all the symptoms of cirrhosis.
The symptoms include dry mouth, tired, bloating,
not hungry, tan looking skin, severe loss of weight, when i bleed it takes a while to stop,
red palms. Why would i have all the symptoms if im not close to cirrhosis. I think the biopsy is not accurate as ive been told it depends on what piece of the liver is taken. Also as far as the
liver functin test, im not doing any damage to my liver now so shouldnt those be normal as they are. Never took drugs, drank alot, took prescriptions drugs for 3 months as stated. Would a hepatoligist be my best option to see. The doctor im seeing is a gasro doctor. I think that he is not as informed as he should be when i ask him questions and give him my symptoms. As you can see every thing just does not add up. My primary doctor says i would only be getting symptoms if my liver was 90% gone. Basically my doctors will not listen or have no answers for the symptoms. Im looking for another biopsy just to see if this one was wrong. In your honest opion what do you think of my hole situation. ive lost close to 40 pds in a year. They cannot explain that either. except i quit drinking, you dont lose that much weight when you dont drink

Dr. A Sun (one of the experts here ) has sent this message ...... "tan skin and bruising are somewhat suggestive of Primary adrenal insufficiency. Maybe a ACTH stimulation test?"
This would mean that you need to get in trouch with your physician for discussing the possibility of a low steroid levels.
The same thing could go with a hypothalamus-pitutary axis dysfunction too,
You could discuss these with your doctor too, this may not answer all your questions but definitely it is a diagnosis to be considered..
The correct specialist for investigating these conditions would be an endocrinologist,

Hi,
Let me rephrase....
The cause of your symptoms could be due to an illness like ADDISON's Disease, that can lead to Tan skin and bruising, along with some of the other symptoms that you have mentioned.
This disease is characterized by a low serum cortisol level and a positive ACTH stimulation test.
A gastro doctor would not be able to diagnoze this condition, and the specific person to discuss the possibility with woiuld be an endocrinologist,

does this have anything to do with someone who has damaged their liver. Or do you think that my liver is fine. Does this have anything to do with someone who has drank alot

Not really anything to do with a damaged liver,
though excessive drinking can sometimes affect the body leading to these symptoms,

drinking can lead to these symptoms and cause liver problems. I think thats why i have cirrhosis, because i have all the symptoms. Well doctor i know its my liver and im looking what i should tell my doctor or tell him to order more tests. What tests should he order at this point.

Of course you are welcome to your opinion, however since you are asking for ours (JA experts), I need to give my opinion rather than just seconding yours.
Given the fact that I am unable to examine and have to rely on your history alone, all i can say is that two facts stand out..
  1. The biopsy done is NOT suggestive of cirrhosis ( and this is pretty much the diagnostic test)
  2. The Liver function tests do not show consistent with the symptoms that you are describing..
I believe that the correct approach would be to repeat your liver function tests 3-6 monthly and if they show signs of significant worsening then further investigations like a repeat biopsy should be planned.
At present I would not recommend a repeat biopsy since this is an invasive test and is unlikely to shed further light ...
