I am a 45 year old very healthy active female that has a concern of a constant hunger no matter what i eat i have had this problem for years i excerise regulary eat whole grains fruits and vegetable watch calories and take vitamins and i stay hungry all day long cant get that full satisfied feeling by mid afternoon i am tired and irratable and i am tired of feeling constantly hungry.

Dear Customer (name blocked for privacy),

There are several causes for excessive hunger, also known as polyphagia. There could be several explanations for this. You mentioned your thyroid was checked. Hyperthyroidism (high level of thyroid) can be one explanation, but the blood tests would have shown this.

Other hormone imbalances could cause polyphagia such as high level of adrenal hormones, called hyperadrenalism. These hormones can also be checked with a blood test.

You didn't mention any other health problems, but often when diabetes is uncontrolled, and levels of sugar are high, a person would feel excessive hunger.

Medications such as antihistamines, steroids, marijuana, or even alcohol can cause excess hunger. Withdrawal from cocaine or amphetamine can also cause this condition.

You may want to check back with your doctor for further blood testing of the hormones I mentioned above. Also having a blood test for diabetes would be appropriate. An FPG (Fasting Plasma Glucose) is usually the preferred test done to diagnose for diabetes or a glucose tolerance test which is also done by blood tests.

I have had hypothyroidism since i was 20 it runs in the family i have had blood work done for years to keep it on track. I have done the fasting gluscose test it is normal i am at a loss dont have drug problems dont drink alcohol i just cant get rid of always feeling a real hunger my diet is very clean i dont eat red meat i get a good 2000 calories a day of good food. and about 500-800 of empty calories a day drink a lot of water and green tea and nothing else

Okay, thanks for the additional info. It sounds like you've got a healthy diet.

(1) Can I ask your height and weight?

(2) Your last T3, T4, TSH if you know it.

(3) Any other blood work? Glycosated Hemoglobin (Hgb A1C)

5 foot eight and 140 and dont know the answers to the other questions i just checked what polyphagia meant and my dad does have type 2 diabetes and i have 9 out of the 15 symptoms of type two diabetes i havent had blood work done in past year and when i work out i get a shakey weak sweaty about ready to pass out feeling of weakness i carry a piece of hard candy with me at all times in case this happens sometimes not always but other times quite frequently

To get a confirmed diagnosis of diabetes, some other tests might be needed. Your fasting glucose can change so much, that it may not be dependable. Even diabetics suffer from low blood sugar from time to time.....so lets not rule that out yet. Your other symptoms certainly point in that direction, too with the shaking, weakness, and feeling like you're going to pass out. Having candy on hand is a great idea because those are signs of LOW blood sugar.

So get to your doctor for testing. The earlier you catch diabetes, the better chance you have of managing it.

For a more definitive diagnosis, your doctor will want to do other blood tests for diabetes such as a glycosylated hemoglobin (Hgb A1C) or the glucose tolerance test. The glucose tolerance test is actually a series of glucose tests over a few hours. First you will have a fasting level drawn; then you will be given a high sugar drink to consume; you will then be tested every hour for 3 to 5 hours to see how your body metabolizes the sugar.
