just need direction please

My father-in-law is approximately 60 years old, smokes a pack a day of Marlboror ultra lights a day and drinks about 1/2 bottle of white wine a day. For the last 4 months the right side of his chest has been hurting. It is centered between his nipple and the bottom of his rib cage; it is worse when he is aggravated. I seen him this evening hold the right side of his chest and bend down. At one point it looked like he might have shingles(but that was not it) It is tender to the touch and radiates around to his back. ( He and I both do not like doctors; so I would like to have at least an idea of where to begin looking for anwsers and know what to ask for his sake)... Please help..... Thank you in advance for your attention and consern.... Terra Smith....
