I HAVE AN APPT. FOR THE FIRST TIME WITH AN INFECTIOUS DISEASE DR. FOR MY 14 YEAR OLD DAUGHTER THIS WEEK. WHAT ARE THE MOST IMPORTANT QUESTIONS I SHOULD ASK (BESIDES THE OBVIOUS), AND WHAT INFORMATION AND DOCUMENTS FROM OTHER DRS. AND HOSPITALS SHOULD I TAKE WITH ME TO THE APPT.

So why exactly are you visiting the infectious disease doctor, is this a routine visit or are there any specific reasons for the visit.
A rule of thumb is that you hsould carry the complete health record for your daughter if that is possible for the visit to any new specialist, you never know what information may be needed,
regards
Dr. Gupta

THE LAST WEEK OF JULY MY DAUGHTER HAD AN OVERNIGHT BOUT OF SEVERE DIARREAH, A COUPLE OF DAYS LATER HER JAW STARTED HURTING SO BAD THAT SHE COULDN'T EAT ANYTHING SHE HAD TO CHEW, AND BOTH SIDES OF HER JAWS WERE SLIGHTLY SWOLLEN. I MADE AN APPT. W/ A DR.. THE DAY OF HER APPT. (THE NEXT DAY) MY DAUGHTER WOKE UP CRYING AND CRAWLED OUT OF HER ROOM BECAUSE HER FEET AND ANKLES WERE EXTREMELY SWOLLEN, THEY WERE NOT BRUISED OR DISCOLORED IN ANYWAY, BUT SHE WAS IN EXTREME PAIN. THE DR.TOLD ME THAT SHE HAD STREP THROAT JUST BY LOOKING AT THE SWAB, SAID THAT THERE WAS BLOOD IN HER URINE, AND THAT HER SED RATE WAS HIGH, BUT SHE DIDN'T KNOW WHERE THE INFECTION WAS COMING FROM, AND THE RHEUMATOID FACTOR WAS NEGATIVE, THIS WAS 08-09-2005, SHE GAVE HER A SHOT OF ROCEFIN IN HER OFFICE ALONG WITH A PRESCRIPTION FOR AMOXOCILLIN, AND TOLD ME TO GIVE HER IBUPROFEN FOR THE PAIN. NOTHING HELPED AND SHE ONLY GOT WORSE (I DIDNT AGREE WITH THE STREP BECAUSE SHE NEVER HAD A FEVER, OR A RED OR SORE THROAT). OUR DR.,A WEEK LATER SAID SHE WAS FINE AND SENT US HOME. A FEW DAYS LATER SHE CAME OUT OF THE TUB WITH PETECCHAIE ALL OVER HER FEET AND ALL THE WAY UP TO HER KNEES, SO I TOOK HER TO THE ER WHERE THEY DIAGNOSED HER WITH POST-STREPTOCCOCAL VASCULITIS C WITH PETECCHAIE AND ARTHRALGIS, THEY PUT HER ON 25MG OF PREDNISONE 3X A DAY FOR 4 DAYS, SAID SHE WAS LOW ON POTASSIUM, AND SLIGHTLY DEHYDRATED,AND GAVE HER IV ANTIBIOTICS, THE ONLY CHANGE WAS THAT THE PETECCHAIE WENT AWAY, BUT WAS BACK AFTER 2 DAYS, AND SHE STILL WASNT WALKING BECAUSE OF THE PAIN AND SWELLING, ALL SHE DID WAS CRY. I TOOK HER A COUPLE OF DAYS LATER TO PHOENIX CHILDRENS HOSP. WHERE THEY ADMITTED HER WITHIN 45 MIN. THEY TOOK 23 TUBES OF BLOOD AND TESTED HER EXTENSIVLEY; AFTER A WEEK THEY HALF HEARTEDLY DIAGNOSED HER WITH REACTIVE ARTHRITIS DUE TO THE STREP, AND SAID IF SHE WASN'T BETTER IN 2 WEEKS, TAKE HER TO A RHUEMATOLOGIST. TODAY (9-13-05)THE ORIGINAL DR. THAT DIAGNOSED HER WITH STREP CALLED (6-7 WEEKS LATER) AND SAID THAT MY DAUGHTER TESTED NEG. FOR STREP THROAT, TOLD ME THAT THEY COULD NOT FIND THE URINE TEST RESULTS AND THAT HER SED RATE AT THE TIME WAS HIGH. I HAVE CALLED SEVERAL TIMES FOR THE RESULTS AND HAD FINALLY GIVEN UP A MONTH AGO.AFTER THIS STRANGE PHONE CALL I CALLED HER DRS. AND MY INS. CO. WHO TOLD ME TO GET HER INTO SEE AN INFECTIOUS DISEASE DR. IMMEDIATLEY. WE ARE GOING INTO THE 7TH WEEK OF THIS AND THE THIRD COURSE OF ANITBIOTICS AND SECOND COURSE OF STEROIDS. MY DAUGHTER STILL CANNOT WALK EVEN WITH THE WALKER THAT THE HOSP. SENT HER HOME WITH BECAUSE SHE CANT BEAR ANY WEIGHT ON HER FEET, THE PETICCHAIE IS BACK AND THE PAIN HAS SPREAD NOW TO HER RIGHT KNEE,MOST OF THE PAIN HAS BEEN IN THE RIGHT FOOT EVEN THOUGH IT STARTED IN THE LEFT FOOT, NOW IT GOES BACK AND FORTH FROM ONE FOOT TO THE OTHER, HER JAW STILL HURTS HER AT TIMES BUT NOT LIKE BEFORE. WE ARE BOTH EXTREMELY FRUSTRATED AS SHE CANNOT WALK, SHE HAS MISSED A MONTH OF SCHOOL AND IS IN CONSTANT PAIN WITH LITTLE TO NO RELIEF EVEN WITH THE PAIN MEDS THEY GAVE HER (5MG OXYCODONE- 30 TABS- 1EVERY 6 HOURS) I HAVE STARTED GIVING HER 2 EVERY 4 HOURS WITH HARDLY ANY RELIEF. SHE ONLY HAS ENOUGH FOR TOMORROW AND OUR APPT. IS NEXT TUES. PLEASE HELP US!!!

THANK YOU,
SCARED MOM & DAUGHTER

I understand your concern and the fact that you are scared,
Looking at the history and the reports that are available, I would go with the diagnosis of Vasculitis.
A postinfectious cause seems less likely since ( as you have pointed out) there is no significant fever.
I would recommned a consultation with someone who specialises in immunology (or even rheumatology as has been done in your case)
AN infectious disease consultation may not be that rewarding, but useful to rule out any obvious infection as a cause.
The prolonged high ESR with multiple systems being affected is a clue to collagen vascular disease.
Please read this link for some more information:
http://www.healthatoz.com/healthatoz/Atoz/ency/vasculitis.jsp

I would recommend a skin biopsy from the areas of petechia and then further investigations and treatment as per the diagnosis
Please get back to me if there are any more queries. Do remember to accept the answer if you find it useful. A positive rating and a bonus at your discretion would be appreciated.
Regards
Dr. Gupta

TO EVERYONE THAT REPLIED TO MY PROBLEM , I APPRECIATE IT MORE THAN YOU KNOW. THIS WAS MY FIRST TIME ON A WEBSITE LIKE THIS AND AM VERY UNFAMILIAR WITH HOW IT WORKS, BUT I DID PAY DR. GUPTA FOR HIS ANSWER. THE FOLLOWING DAY WE WENT TO UMC IN TUSCON TO SEE THE INFECTIOUS DISEASE DR. AND MY DAUGHTER WAS IMMEDIATLEY ADMITTED TO THE HOSPITAL.

THEIR DIAGNOSIS AFTER COUNTLESS TESTS, AND A PUNCH BIOPSY WAS FOUND TO BE LEUKOCYTOCLASTIC VASCULITIS. IT IS THOUGHT THAT SHE WILL BE IN TREATMENT FOR THE NEXT 6 MONTHS, AND THAT THE DIAGNOSIS IS LIKELY TO CHANGE THROUGHOUT THE COURSE OF THE DISEASE.

AGAIN I APPRECIATE EVERYONES HELP WITH THIS PROBLEM. I HOPE I HAVE DONE THIS THE RIGHT WAY AS FAR AS PAYING PEOPLE,( I REALLY DON'T HAVE A CLUE WHETHER I DID OR NOT). PLEASE LET ME KNOW ONE WAY OR THE OTHER . MY INTENTION WAS NOT TO CHEAT ANYBODY OUT OF ANYTHING. WE GOT HOME LAST WEEK AND I HAVE BEEN FIGHTING WITH THE SCHOOL ABOUT HER ABSENCES, TRYING TO SET UP A HOMESCHOOL PROGRAM,DR'S. APPTS., ETC...

I TRULY DO APOLOGIZE IF I HAVE NOT RESPONDED THE RIGHT WAY.

THANKS AGAIN,

KIMBERLY MOORE
