There are two conditions you could be suffering from:
- Arthritis of the sterno-clavicular joint. This is the joint between the sternum (breastbone) and the clavicle (collar bone) at the base of the neck. Since you already have arthritis of other joints and there is swelling at this region, I believe that this condition is the cause of the pain and the inability to raise the arm.
- Costochondritis: This is a condition where there is pain and inflammation of the joint between the sternum (breastbone) and the ribs - in your case, the first rib. The symptoms are similar to the previous condition - pain, swelling, difficulty in raising the arm etc.
I believe that you need to be examined to diagnose the cause. Xrays and a physical examination should be diagnostic.
If it is costochondritis, you need to rest the arm and take a painkiller like Motrin 500mg twice daily after food for at least 5 days. You should start feeling better in about 1 weeks time.
If, however, you are suffering from arthritis, besides the Motrin you will also need to take medicines specific to arthritis. Your doctor will be the best person to decide what you need to take after examining you.
