What is this 15-year-old going through?   A 15 year old with type 1 diabetes is brought to an emergency room by his parents for evaluation and treatment. His mother states that 36 hours prior; he began complaining of a sore throat, fever, lethargy, increased thirst and increased urinary volume. He was treated conservatively at home with Tylenol and bed rest, but continued to worsen. As his condition worsened, his oral intake of fluids dramatically decreased, glucoses skyrocketed, and he slept almost constantly.
     Upon arrival at the emergency room, his skin turgor was poor, his mucous membranes dry. Ketone-tainted breath was noted, and his capilly glucose was 824.
     Vital signs revealed a pulse of 120, blood pressure was 80/50, temperature was 103.5 degrees and respiratory rate was 40.
