my nephew is in the last stages of cancer, completely bedridden, and on morphine for the pain; when they changed his diaper yesterday the urine was black, what does this mean?

Black urine can indicate bleeding high in the urinary tract which has then been changed by the acidic urine. This can be an indication of kidney damage.

However some medication can also cause urine to become black-brown. You need to have him checked by a doctor and he will need some blood tests to look at his renal function as a matter of urgency.
