A ct and ultrasound, lots of blood work and the tumor is 6 cm blocking both bile ducts. We can get release of records. Where do we send them?

We would like a second opinion and would like to send our records to Mayo Clinic. How and where would we go about doing this? Thank you. Penny Clemence

First, you have to contact the doctor at the Mayo Clinic that you want for the second opinion. After you have chosen a surgeon , you will have to sign a medical release form. Find out the fax number of your chosen surgeon's office. Go to Dr. Chung's office and sign a release form and ask them to fax your record to the 2nd opinion Dr's office. I would also have them make you a copy of your record that you could hand carry to the Mayo Clinic with you, when you are seen for a 2nd opinion. I hope that this was helpful.
