An overlad of fluids can cause cngestive heart failure in someone who has a weakened or poorly functioning heart. Generally though, congestive heart failure will not cause chest pain. If someone has coronary artery disease, which is a blockage in the blood vessels of the heart leading to a heart , then congestive heart failure from fluid overload can put an additional strain on the heart which may precipitate a heart attack.
The enzymes you are referring to for a heart attack are CPK and a specific form of CPK called Troponin. CPK can increase even with an injection in the muscle, but Troponin is more specific for the heart muscle.An EKG can also tell if someone had a heart attack. Very small heart attacks may only show up in the Troponin and not on an EKG.
Giving a lot of fluids for high fever and low blood pressure is the right thing to do. Often it is hard to tell exactly the right amount of fluid unless there is a special catheter in a large blood vessell to monitor the fluid pressure. Very small heart attacks are not uncommon in an older person who is very ill from something else like an infection.
