signs of kidney infection

I am having pain in my back in the area of my right kidney. I am not sure if it is a pulled muscle as I have never had kidney pain before. The pain is not constant and hurts only when I am lying on my back or left side and when I caugh or clear my throat in these positions or if I put pressure on the area with my fingers it seems tender. Also if I have to urinate, the pain does not go away but is less intense after urination.

Already Tried:
Nothing, I am diabetic and am not sure if I should try cranberry juice or capsules as suggested by family or if I should proceed to the ER - pain is about 6 on scale 1-10, 10 being the worst pain I have ever felt.
