Can you tell me which part of the NHS has been involved in your care (GP, dentist or hospital etc). Can you tell me what part of your treatment has been poor?

Also what is your approximate area, we need to know what Primary Care Trust you fall under.

Finally, who have you tried complaining to so far? What has been the outcome?
