Pain Urinating

HI I find it hard to urinate in the morning as soon as I wake up.I have to exert a lot of force around my bladder.It is not painful but i need a lot of force to pee normally.I also feel the pressure with which my pee comes out isnt as big as before.Also as a 22 yr old man,I do masturbate and lately I have felt it painful when i ejaculate.During a routine urine test last week,my doctor said there was bit of Blood in in urine...I do not know if the following sympytoms are related but they are: lower back pain,upper left back pain (on deep inhalation only)ie heart region.

I have had little blood in my underwears since about two years,but i thought it was too little and notmal.It is constant as everytime i change my underwear I notice it.The back pain,I mainly feel in the morning when I wake up.I was confused at first between bad sleeping posture,but i sleep flat now and still have same problem.it’s a squeezing/dull pain.It’s worse when i protrude my belly.I have taken paracetamol,but this is a chronic problem,since about a year..

I m chronically stressed,depressed,and sometimes cannot control my tension and worries,ie i get anxious.My blood pressure is normal.My mum has diabetes diet controlled,cervical spondilitis and my dad has high cholesterol and hypertension.
I am a bit worried about the urine in my blood.I would like to know the causes,and treatment(is it invasive,)
