Is it harmful if a child swallows a gel cap with a foam shape inside that swells when disolved in water?

Hi Saling. I would not take a chance on this one, some of those toys are made with a sponge like material and I would guess it is NOT safe. Please call your Doctor now! If you child is that young, they can give him/her a suppository and then you have to watch to see that is comes out. This is not something to guess on. If your Doctor is not available, please go to the ER. It is always better to be safe than sorry. Let me know how you made out!
