The prostate exam and PSA are quite good for determining prostate problems. That part is probably OK.
There are several sets of tubes which come in contace with or are involved in the production of semen. The testicles, vas deferens, cowpers glands, seminal vesicles and the semen eventually passes through the urethra (common path for urine and semen). Abnormalities at any of these spots can cause bleeding. Painless bleeding witho9ut fever, swelling etc. is a strong suspicion of a tumor/cancer. I do not want to scare you, but as physicians we treat for the most likely problem(in this case your physician felt infection was most likely) OR we treat for the worst possible problem(in general that would be a cancer)
The tests are simple and non-invasive. The most invasive would be a cystoscopy which is a tube inserted though the penis into the bladder to visually inspect the interior of the bladder. If the blood is NOT in the urine and only in the semen, they may not even do this.
Nevertheless, be sure to contact the urologist promptly and tell him/her that the blood had NOT improved even though you have been taking the medication regularly and ask what is the next step.
