pinched sciatic nerve

I have a pinched sciatic nerve (L5 is what was on the test results EMG in 1991. I now have permanent damaged most of which is in the inside of my knee, which is so tender I can not even touch to put on pain creams. I have had steriod shots a couple of times, helps for a short time and my doctor said I could only have every 4 months. I have asked for a pain patch (Lidoderm) and he said it is only for chornic pain, which is what I have. I can no longer run, walk for excerise. Sometimes the pain is so bad I can hardly bear it. Sometimes the pain in my kneecap hurts so bad, it burns plus the pain. There are times the pain goes all the way down to my toes. Ibupbrofen does not help even at a prescription does (4) tablets. What suggestions do you have that may help. I have been through extensive therpy, so far nothing helps for any lenght of time. In 1991 the my Back Surgeon said no operation. I am desperate.
