If a person has been taking oxicontin 80mg 4 every 6 hours what are the side effects. He has been on this dose since 2001 and what are the damages done to the body and the brain.

At this dose he would have developed some tolerance of drowsiness and confusion. He may well be suffering from chronic constipation. It may have had an impact on his liver but only a Liver Function Test can confirm this.

The more concerning issue is that he will be both physiologically and psychologically addicted to the medication. What he must not do is stop this abruptly, if he is concerned about ongoing use then he needs to speak to a doctor about cutting down his dose gradually over several months.

The other thing to consider is the initial cause of pain, has that been resolved now?

He takes it for back pain I don't think he would stop I have tried to get him to try something different he won't if he sleeps in a reclining position he swells from the knees down to his toes and his short of breath when he showers and he sleeps all the time when he is not working I have noticed lots of personalty changes in him in the last 6 months he seems to be going down hill how long can a person go like this before it would cause a heart attack or death.

Regular long term use can have an impact on the cardiovascular system and respiratory depression. From the description of leg swelling, shortness of breath then he could be suffering from pulmonary oedema (fluid around the lung) or heart failure, this is where the heart is not working so efficiently. In severe cases this can lead to death within 5 years but the earlier it is treated then the better the quality of life. I would suggest he sees a doctor for a cardiovascular work up. At the very least he will need diuretics to start with then possibly other things to help his heart or lung function.

If he is going downhill then you need to urge him to see his doctor, as the symptoms need to be treated. Stopping the oxycontin at this point would not have significant results, the most important thing is to treat the existing symptoms before his painkiller is reviewed.
