To Dr. Mazumdar....You've explained the procedure of an upper vaginectomy to me before.... but I'm going in tomorrow morning for a simple vaginectomy, can you tell me the difference as I know this one is an outpatient procedure and you said the other one is a 3-5 day stay.   Thank you.

In Upper Vaginectomy, only the upper 1/3 rd of the vagina is removed. The patient needs to be admitted the day before the operation for pre-operative preparation. Once the operation is over, a vaginal pack (made of gauze) is inserted and kept in place for 24 hours. It is removed the next day and the patient is allowed to go home only after ensuring that there is no bleeding from the vagina.

In simple vaginectomy, the entire vagina is removed. Here, we normally admit the patient the day before the operation, but I guess she can be called in the day of the operation also. Once the entire vagina is removed, the open area is closed off by stitches, ensuring no bleeding. The difference in upper vaginectomy lies at this point. Since a part of the vagina is left in place in upper vaginectomy, a pack needs to be inserted to prevent bleeding. Since in simple vaginectomy, the entire vagina is removed, no pack needs to be inserted. Normally we keep the patient admitted in the hospital for at least the first 24 hours to keep her under observation. Perhaps it is different at your hospital.

Out here, we do not do simple vaginectomy as an out patient procedure. The patient needs to stay in the hospital for 3-4 days, since we admit her the day before the operation and keep her for at least another 24 hours after the operation.

Reply to Dr.M.D.Mazumdar, MD's Post: Sorry, I should have said a "simple partial vaginectomy".   He told me it was an easier version of it, I'm just trying to find out what makes the difference and is it still going to hurt as much?   Also, he said I should be able to go back to work after a couple days and I'm trying to find out if that sounds reasonable or should I really expect to be off more.   I really like this dr., totally different from the 1st dr. I told you about... you may not get a reply in before I leave for the hospital so thank you for any help you can give.

Yes, a 'simple partial vaginectomy' is an easier version since only the affected part of the vagina is removed. A special solution is used to outline the affected part of the vagina and this affected part as well as a little normal tissue near it (to be sure that even microscopic cells have been removed) is removed. This leaves the vagina more or less intact (assuming that very little tissue is involved)

In Upper Vaginectomy, the entire upper third is removed, regardless of how much tissue is involved. So occasionally, more tissue than necessary may be removed.

Ths doctor sounds quite knowledgeble about the subject. I sincerely hope you the best during the operation.
