I recently received a report from my doctor that was sent to the hospital authority board due to my injury on duty. It stated that I was taken off my elavil and put on lyrica but that is a false statement. I have been prescribed 3 meds from my pcp and my back surgeon added the lyrica in conjunction with those. He refuses to change the report. What can I do legally to get this changed?

Could you please give me more details about the injury? Why you were on the Elavil/Lyrica? Why you saw this doctor? Why was the report sent in the first place?
Why does the doctor not want to change the report? Why is this inaccurate report a problem?
