Question

nursing error

I am registered Nurse. I went into a home and they told me that the nurse before me was taking IM medicine and giving it PO. I thought it a little strange but we do strange stuff in Hospice nursing. It does not have any adverse affects I know doing this. But now I need to show some sort of evidence that my nursing judgement is not in question. Can you find any documented material where this has been done. and it was a positive thing not negative.

Does the Physician's Order call for the medication (normally given intra-muscular) to be given orally -- if so and if you 'know [that administering the medication orally] does not have any adverse affects' [t seems tp me that you are covered.  If in doubt check with the Doctor.

What is the medication?

The handwrittten order written on a TO said. Haldol 2-5 mg PO or IM Prn agitation. Same thing thing with ativan.   One error was not searching the home for a PO medication. Both were in the home. The woman had seen the nurse draw up the syringes and she had been there when the PT took the medicine in cranberry juice. What the woman didn't know is that the IM was not nesccary since he took the PO she just waited the IM. So when I saw the two different vials missing and she said with out a doubt that this was the medication given I figured well the nurse just put it in the cranberry juice. My supervisor is having me for lunch right now. She wants me to prove why I would do something like this. I know we have put PO med in inhators and thats sounds weird too.

Your reply is a bit unclear as to which medication(s) you administered PO. Still, given the fact that the Physician's Order authorizes either IM or PO administration, it seems that the method of administration is clearly left to your professional discretion.  Even so, your supervisor wanting an explanation of why you chose PO administration seems reasonable.

Please let me know how it went with your Supervisor.

THere was 2 sets of medications in the house. one being oral the other IM. I did not know there was Oral medications in the house when the family member told me that she was sure the nurse gave the IM medications PO. My nursing supvisor told me I should have called her if I questioned giving IM medications PO. But if I can prove with litature that IM solution can be givin PO then we will attach it to the incident report and it will look more favorably in my personel file. Both medications ativan and haldol were to be give together.
