MY DAUGHTER HAS BEEN COMPLAINING OF STOMACH PAIN CONSTANTLY FOR THE PAST 2-21/2 MONTHS IN APRIL SHE WENT TO THE ER BUT WE CAME BACK WITH NO ANSWERS THEN WE FOLLOW UP WITH HER PED. HOW SAID IT COULD BE MENTAL BUT HE REFERRED US TO A GASTRO. THE GASTRO GAVE HER A STOOL SOFTNER EVEN THOUGH I TOLD HIM SHE GOES TO POO EVERY DAY. SHE NOW HAS A FEVER, STOMACH PAIN AND LEG PAIN. I ASKED FOR A COPY OF HER BLOOD WORK WITH SHOWS ALKALINE PHOSPHATASE HIGH AT 305. NEUTROPHILS LOW AT 44.1 LYMPHOCYTES HIGH AT 46.5 AND MONOCYTES HIGH AR 6.9? ARE THESE OF ANY IMPORTANCE AND WHAT SHOULD I DO? I FEEL LIKE NOBODY IS TAKING THIS SERIOUSLY SHOULD I BE WORRY? MY DAUGHTER IS ONLY SEVEN AND SHE IS THIN AT 48 LBS. PLEASE HELP

Dear Jessica,
I am a pediatrician. I understand your anxiety and have seen quite a few similar cases, with as worried parents as you have been.
At your daughter's age, 'functional' stomach pain can be an important issue if other causes have been ruled out.
The red flag (warning signs) that can indicate a serious cause (organic) are the following...
  • waking up at night with pain
  • vomiting
  • loss of weight and appetite
  • high persistent fever
  • accompanying loose motions
If none of these are present and the child is otherwise active and playful, then doing too many tests may not be productive.
Please understand that the child is NOT LYING, she is feeling the pain, but the cause is not in her stomach, it is in her head.
Similarly it does not necessarily indicate that you are a bad parent, the child can have very trivial (to us) reasons for this sort of attention seeking behaviour.

Coming to the second part of your question about her present problem, this is NOT (in all probability) related to her constant pain problem over the last few months
I belive that the reports are suggestive of a possibility of a viral infection, however a visit to her pediatrician/ ER maybe necessary if she has severe pain.
Alkaline phosphatase maybe elevated in kids since it reflects bone growth ..

The lab reports are not conclusive for any particular cause, probably suggestive of a viral infection, (increased monocytes, and lymphocytes).
As I said before, the serum alkaline phosphatase is probably normal...
THe marks on the neck are probably not related and maybe related to some sort of a dry skin condition...
Has her abdo pain been always associated with fever...? IF that is so then a diagnosis of functional abdo pain is quite unlikely and she probably requires another gastro opinion...
