Dear Customer (name blocked for privacy),
There are SO many causes for headaches, and sometimes it's just process of elimination to finally diagnose the cause. More common causes for your headaches may be stress, diet related (a lot of caffeine), or even sinus infection could cause headaches. I want to share with you a site that discusses tension headaches. Tension headaches for many people are chronic. This site discusses treatments and diagnostics, too.

You asked if neck injury could cause headache......absolutely. Whiplash is one cause. Degenerative disc disease could also cause chronic headaches if the affected discs were in the cervical spine.
Mepergan is meperadine (Demerol) mixed with an antihistamine (promethazine.) The promethazine combats the nausea that often occurs as a side effect of Demerol. The tramadol is not a narcotic like the Mepergan, but the action of it is a lot like an opiate, so it too is a pretty strong analgesic. Between the both of these drugs, your headaches should be controlled. Be aware that BOTH tramadol and Mepergan are highly addictive and are not intended for long term use.
Again, please refer to the above site. Perhaps consider some relaxation exercises to reduce any stress, avoid caffeine, and look into a possible sinus infection. Take comfort in the fact that the MRI would have shown any tumor or mass in the brain.
