what is he at risk for? what could be the cause of swelling

my friend is on pain managment. he takes avinza 30 mg daily , lasix and blood pressure medication as well. he also takes xanax 1mg hs. he cant seem to get his pain under control. his ankles and feet swell to about twice their normal size, oh he also takes 10/325 norco, 1q 6h prn, but the real problem could be the fact that he drinks a half gallon of vodka every 2 days w/ tonic water. he is about 75lbs over weight and is only 46 yrs old. last year he had a hip replaced. he has lost all his top teeth and wears dentures. he sleeps about 10 hours daily , from 2am until about 11:30am. he eats around midnight before bed. His pcp also has him on prilosec. he is unable to work and is on ssi for psychiatric reasons. I told him if he does not stop drinking he may develop a GI bleed. he complains about his condition constantly. but does not seem to think the alcohol is a factor for his swollen ankles and feet. when he takes his noro he seems to have a few hours of energy. which causes him to clean in an ocd fasion. what could be causing his legs to swell. he also smokes 1 pack of cigarettes per day. Could you please give me some feedback
