wife has fever of 104 with rapid heart rate of 115. gave her tylenol for fever. should she go into ER? What is this?

If the tylenol is not bringing down the fever, it merits a trip to the ER or a call to the MD. It could be the influenza if she also has upper respiratory stuffiness. If not it can be anything from a kidney infection to a blood infection called sepsis.
