Heart, lung or anxiety problem

I an 38 years old, I have been running a body shop for 15 years, I an exposed to isocionates in the shop. I have been a daily pot smoker,sometimes when i work too hard it seems like I am havng a heart attack, if I sit down it goes away. sympoms are tightness in the chest, dizzyness, heart beats hard, I feel like I am going to pass out, if I move around too much it gets worse, it has gotten to the point where i cant do anything .the other night it got bad and I went into the ER, they ran bloodwork, and a EKG, they checked my thyriod, O2 levels, colestoral, blood sugar, the blood sugar was a little high (I had eaten supper about 2 hours before) the EKG turned out good, they think it is anxiety, they started me on 10mg of paxil (pexeva) at night before I go to bed, (2 days ago now) they also gave me Niravam for when I have an episode come on, .5mg it seems to help some, I was back in today and the doctor dosnt realy know whats going on, can you come up with any ideas,

Reply to Dr. Steve's Post: I have had small episodes for some years, since age 19, I allways thought it was from overworknig myself, the first bad episode was about a week ago, the symptoms came except this time I wasnt working, it would come and then go away and then come back agian, this weht on for about 2 hours. the paxil, tonight will be my third night, (havnt taken it yet) . my grandfather died of congestive heart failure at 86, my mother is very healthy and young lookin for 60. my father died in a car accident at 40, he had high blood pressure, I have a little nephew age 12 who has anxiety, other than that everyone else in the family seem normal with no problems, even going back to great grandparents there were none of these problems.

The MOST likely Dx is: Acute, Recurring Panic Disorder (more serious than just "anxiety" or "nerves"...it can mimic heart attacks, or stroke sx).
