Sinus rhythm or arrhythmia?

Sent to Health Experts October 25 08:40 PM

Hello,

I have recently had a holtor monitor on as they are investigating the reason behind why I have been having palpitations and SOB, also tiredness and fatigue. I am a 29 year old female.

The holtor monitor came back with sinus rhythm with some sinus tachycardia. I have had some cardiac experience as I am a Registered Nurse.   Last night I was looking at the rhythm strip and it seemed to be irregular (there wasn't always the same distance between each QRS complex) , also the T segment was hard to identify. I am finding it hard to believe that this is sinus rhythm especially when I am so symptomatic.   Please help

Thanks
Louise

Hi there,

i have a lot of experience with reading EKG's or rhythm strips. You could have a sinus arrhythmia this is normal and it happens as you take breaths.

The fact you have a flattened T wave isn't that concerning. It can be normal and dosen't mean that there is anything wrong.

If you want you could fax the strips to me and I would look at them. I could tell you what I think. I would be glad to look at it for you.

If you think it is irregular you would think about atrial fib or a heart block. so look for p-waves before the QRS complex.

If it is dropping a QRS complex that is a problem as well. I would be looking for PVC's or irregular beats that my be causing your PVC's

Let me know if you want to fax the strips to me.
Be glad to look at them. Wrote me back if you can send them

Thanks
Dan
Hope this helped.

Thankyou for that. I will get my husband to fax them to you. What is your fax number?

So would this sinus arrhythmia be causing the palpiations etc etc?

Unfortunately my husband is unable to fax the rhythm strips as they have a block on their phones restricting international calls. Do you have an email address that we could send them too

Thanks
Louise

Hi Louise,

I have looked at your rythmn strips. I also had any girlfriend look at that who is a ICU nurse. We both aggree that there is nothing irregular with your rythmn strips. The assessment od sinus ryhtmn with periods of sinus tachycardia looks to be correct.

My only theory on your palpitations is that maybe your are having them when your heart rate goes up. it could be possible that you get anxous or nervous which causes your heart rate to go up then you feel palpitations.

This has happended to me when I have panic attacks. Just though maybe you should consider this. Panic attacks could account for your SOB, and chest pain. Just thought maybe that could be a consideration.

let me know if you have any mroe thoughts. Be glad to talk to you more.

Dan
