Thanks for the additional information.
I think there are a few potential things going on here, but I think also that you need to have this evaluated soon, if not immediately.
Testicular pain or "heaviness" like you describe can most certainly be a symptom of testicular cancer, and this is why you need to have this evaluated immediately. Sometimes a mass is palpable, but not always, so this really needs to be exluded first and foremost.
A hernia is a possibility, but if there is no mass present, and no other physical abnormality that you can feel, then this might be less likely. Hernias can involve the protrusion of intestine into the scrotum. Since you can't feel anything, however, this might be lower down the list of possibilities.
Other causes of scrotal pain/discomfort include abnormal blood vessels in the area, and even abnormal collections of urine. Infections of the epididymis (the small structure adjacent to the testicle) and testicle can also be involved.
But first, and foremost, tesicular cancer has to be ruled out as a potential cause. So you should get into see someone as soon as is possible. Don't wait.
I hope this helps. Please let me know if you have any further questions - I'm more than happy to help.
Take care - Dr. Fay
