I am afraid that you have had a recurrence of the inguinal hernia. That is why the surgeon is shocked.
I am sure that you had a mesh put in the abdominal wall during the second surgery. The hernia has now occured at a point higher than the mesh. It is possible that either there was some amount of infection or that repair was not adequate.
The CT scan will look for the point of opeing in the abdominal wall, whether any gut ispresent in the hernia sac , the position of the sac relative to the mesh if a mesh was used etc.
I am sorry to tell you that you will need to undergo another surgery to repair the hernia.
