Without some tests there is not way to pin point this. Since this started after picking up 75lbs.. it may be that you broke blood vessel with the pressure that can occur from lifting (if you didn't life this item properly) which is what could be causing the pain as well. Or this could be a coincidence that it's just occurring the same time, which would indicate possible infection. I can certainly understand your frustration of going through all you went through last time, just to find out there was no stone. I don't know how long ago this has happened. If you are not having any trouble urinating or burning etc.. you may want to wait a day or two and see if it does get better. Which drinking plenty of fluid in the mean time. If the pain doesn't get better then you should be seen as well. Just to make sure nothing is being pinched off internally etc.. I would suggest being seen though ( I would feel more comfortable) but you know your body better than anyone.. plus you know how you lifted the object etc.. So without further testing, it can't be pin pointed at this point.. Does the pain feel like it's getting better or getting worse? Is the blood in the urine changing in anyway?
Either way it's a tuff call.. I would say get it checked out.. just to make sure there is nothing serious..

OK. that I think at this point takes away possible injury of lifting the item. But it also helps in knowing there is testicle pain. Most common causes for testicle pain is: Injury, Infection or inflammation, Testicular torsion.

I would at least see your doctor for a urine culture to see if there is an infection. Which if there is, it can cause all the above. It may just be a total coincidence with lifting the box. If it were a stone.. Most men are in serious pain, so I wouldn't think that at the moment.. but I would bet there is an infection going on.

Hope this helps.
