My question pertains to STDs. Is it possible to get an STD from masturbating with the underwear of an infected person?

Yes if there were any bodily fluids on the underwear then it is possible to get an STD this way as most underwear carries the discharge of the person who has worn it so if you use it to masterbate then it is possible to catch an infection this way. It would be more rare then actually having sexual intercourse with them but it is possible.

How long can the STD survive on the worn clothing?

It would survive possibly as long as the worn clothing was not laundered but it would decrease in possibily if the clothing was a couple of days old and the body fluids were dried. Keep in mind it is still possible but the longer they have not been worn by the infected individual the lesser of a chance of transferring the STD.
