It is likely related to the orapred. Though oral prednisone tends to cause edema, which is water in the tissues the aptient is also sometimes dehydrated. The edema is due to increased sodium drawing water from the system and into tissue. The water in the tissue becomes non circulating and symptoms of mild dehydration appear.

Another side effect is decreased wound healing, thin, fragile skin, and decrease immunity. This could allow a yeast infection to begin on both the feet and the hands.

He should see his Doctor who can evaluate for dry skin vs. yeast and prescribe either a cortisone cream or anti-fungal.

There are side effects of kidney disease also causing dry cracked skin, so certain lab tests may be needed.

In the mean time keep his hands and feet very clean to prevent infection. He should wear only white socks, change them at least once daily and go barefoot when possible.

Good luck, let me know if I can do more.
