This can be due to a kidney stone that may have passed into on of the ureters or into the urethra. Or it could be a bladder infection. Blood may mean that a stone is irritating the linings of these areas or that the infection is irritating the bladder wall. The amount of blood and/or of pain may determine the urgency. The problem with blood is that if it clots in the urethra he may be unable to pass his urine. This can lead to major discomfort as you can imagine. A kidney stone can lodge anywhere in the urinary tract and can also cause obstruction.
Not only are obstructions painful, but the kidney/s can be damaged if the pressure backs up causing what is called "hydronephrosis."

I would suggest that he be seen in the emergency room for a good eval - they will likely do an Intravenous Pyelogram to rule out stones and a urine specimen for culture to rule out infection.

Lower back pain can be either of the two as well. Generally back/flank indicates kidney pain and the lower pelvic pain would be more related to the bladder, urethra, etc. It is possible that he has multiple stones in transit through the system or that there is what we refer to as "gravel." This is small articles that come through over a period of time and can be irrititating different areas at the same time. Push fluids orally and encourage cranberry juice if you have it onhand - otherwise water, tea, etc. will be fine. Is he having any problem with passing the urine?

There is also another possibility and that is prostatitis. This has symptoms similar to bladder and kidney infection. My knowledge of this disorder is less so I have included a link to the following site!

I will be working on other customers questions as well, but I will check back to see if you have any other needs shortly.
