testical swelling and soreness question

I am not sure of how to ask this, but my boyfriend for the last 7 days has been experiencing pain and slight swelling in his left testical. He does not have health insurance therefore he is being very stubborn on going to the doctor being we can not afford to pay a large sum at one time. What are the options, if he goes to the hospital about this will the hospital bill him or demand payment right there. What might this problem be? Is there anything over the counter for this type of problem?
