There is no clearcut demarcation that one procedure is superior to the other. Both have their advantages and disadvantages.
- In reconstruction - the main advantage is that the strenght of the ligament constructed is maximum. Also since it is a bone ligament reconstruction the ends of the constructed ligament are very strong and donot give way inlater years, which is adrawback in other procedures. The main drawback of reconstruction is that there is a risk of patellar fracture and osteoarthrosis, difficulties in achieving full extension of the knee, and the potential for long-term pain in the patellar tendon and patellofemoral joint.
- Cadaver graft - Its main adavantage is that there is amble amount of tissue that can be grafted. Also no extra incision sight will be there on your leg for taking a graft. Only disadvantage is the risk of transfering infection from the cadaver to you like HIV and Hepatitis B. Normally the graft is kept frozen, which should kill all the organisms, but there have been very few cases of transmission of infection.
So the pros and cons have to be weighed before taking a decision.
