Severe Tenderness around the breast area and nipples both sides no redness, no swelling, no lump can be felt. Cand hardly stand the touch in these areas. We are talking a male, 81 years of age and is diabetic with Pacemaker.

The breast itself is tender not the ribs or muscles area. Both breast are affected. No enlargement and no discharge from the nipples. There is no pain when the arms are moved up and down or in bending the body side to side.

The most common causes of severe pain in the breasts in elderly males are infection and breast cancer. But both these causes usually occur in one breast, not in both at the same time. Breast cancers also usually cause a lump in the breast while infections cause redness, discharge etc.

Causes that can affect both the breasts are usually hormone related. At your age, the level of testosterone will be quite low, leading to a corresponding increase in the level of the female hormone estrogen. This can often cause growth of the breast tissue with pain. Although you have not noticed any enlargement of the breasts, it is likely that a mild enlargement is present.

Another cause of bilateral breast enlargement in males can be due to high levels of the hormone prolactin. Prolactin too causes slight enlargement of the breasts which may cause pain. Since you already have an endocrinological problem like diabetes, you may well have other hormonal problem also.

Please consult your doctor and ask for tests of hormones like estrogen, testosterone and prolactin. You will also need an ultrasound of the breasts.

Meanwhile, please take an OTC painkiller like Motrin or Aleve, 500mg twice daily after food.
