I seem to have an extreme intolerance for alcohol. On occasion, when I have not eaten anything, just one drink and sometimes even less, will make me feel ill the following day. I guess the solution is not to drink alcohol at all, but does this indicate a medical problem?

No it does not actually mean a medical problem, especially if you have this symptom when you are drinking on an empty stomach. This allows the alcohol to go straight into your bloodstream which can have a greater effect on your brain and body. It can also have alot to do with the fat and weight content in your body. If you are a smaller person then it will take alot less for you to feel the effects of the alcohol. You also may have an intolerance to alcohol and if you drink less then one drink, have a full stomach and feel ill the next day then I suggest that you do consult with your physician and have them run some bloodwork to determine why you have such an intolerance to alcohol, it could be that you have the beginning signs of an ulcer and they can run an x-ray to determine if this is the cause.
