I am a 46-year-old pre-menopausal female.

I am a 46-year-old pre-menopausal female. I have a 7mm right-sided pituitary adenoma. A neuroendocrinologist diagnosed me with "central hypothyroidism" and hypopituitarism. I have low free T4 and TSH despite being on 75mcg. of Synthroid for the last 14 months. A DEXA Bone Densitometry scan performed 10/21/05 IMP: 1.Osteoporosis with T-score -2.7.

In May-July of 2004 results of urinary free cortisols were 72 (H), 23 & 20 with a normal being between 0-50. In 09/04 a Free cortisol urine of 9.5 ug/24hr normal 10-34, Free Cortisol Urine 1.0 ug/dL, Free Cortisol/Creatinine Ratio 17ug/g norms between 9 - 32. Also in 09/04 the 17-HYDROXYCORTICOSTEROIDS, URINE result was <1.0 mg/24h norms between 2 - 6, 17 - Hydroxycorticosteroids, Urine <0.1 mg/dL, 17-Hydroxycorticosteroids/Creatinine Ratio result <1.7 mg/g with norms between 1.6 - 3.6.

In May, 2004 I had an ACTH level of 88, 02/24/06 result of 7pg/mL (5-27).

I have no pituitary specialists in my area and have been without health insurance, which fortunately will change on July 1, 2006.

I was wondering what your thoughts are on my lab results and where and how do I get proper help. Also what do you think the elevated Intact PTH level may indicate in correlation with my ionized calcium results? Thank you for any input you may have.



My calcium levels are pretty consistently in the upper range of normal, my potassium level on 02/24/06 was 5.3 H mnol/L (3.5 -5.1), Sodium 135 L mmol/L (136-145).

while i still had medical insurance i made a trip to California and was tentatively scheduled to have surgery. I had all pre-op testing, met with the neuroendocrinologist and pituitary neurosurgeon several times. I spent 2 weeks there and the Friday before the Monday of the scheduled surgery the endo said that he wanted to wait on surgery for more testing...that he thought my problem was just Growth Hormone deficiency

There is an Alk. Phosophatase level result of
100 IU/L (25-150). Also Alkaline Phos. Isoenzymes:            Result Ref. Range
        Liver fraction 23%L (26-86)
        Bone           66%   (11-68)
        Intestinal     11%   (0-16)

Reticulocyte Ct.        0.4% L (0.5-3.0)

By HGH I presume you are referring to Growth Hormone. The neuroendo in CA measured that as IGF-1 (BL) and the result was 114 ng/mL. The ref. range was charted by age, mine being as follows:
