How would I obtain my medical records from 1975 ...

How would I obtain my 1975 medical records from a hospital in Orlando Florida that used to be known as Orange Memorial Hospital? The hospital of that name no longer exists.   I was 16 in 1975 and had a baby that was privately adopted through the practice of my OB/GYN. The obstetrician’s name was Dr. W. B. Cheslock. The phone number of his practice at the time was 843-7612 and it was either located on Orange Ave or Orange Blossom Trail in Orlando.

I was given petocin to induce labor and was sedated while giving birth. I was asked to sign over the adoption papers before I had fully recovered from sedation. I had asked to just see and hold the baby. No one was in the room with me at the time and the nurse was rather forceful.

I would like my records because I can't remember the day the baby was born but know it was in October 1975.   I recovered on the surgical floor not the maternity floor. The child is now a 30 year adult and I would like to register my information along with the date of birth and sex in case the person is looking for their birth mother. However, I can't do much without the date and sex of the baby. I am now 47 years old and live in the Chicago area. How would I go about obtaining my 1974 hospital records, where did the hospital move, and who is the best person to contact at the hospital?

PS - The birth father (whom I have not kept in contact with) has been in jail for many years in Florida for sexual abuse and violence. He was not interested in the child when I was pregnant nor when the baby was born.

Orange Memorial Hospital is now known as Orlando Regional Medical Center. They most likely have those records archieved and you may be able to request a copy through them. See link below.

You may be able to have better results by going through the vital records of live birth with the State of Florida. But, it would be necessary to get the exact date of birth through narrowing down the time by your hospital stay. The sex of the baby will also be necessary.

Usually, its done through the mail and address the request to the Medical Record Coordinator. They run their own department and is actually quite organized.

I would call the center in the morning, to see if they require certain identification or other information before making the request. It varies depending on the state and hospital. With HIPPA laws and privacy acts, they may have set policy already outlined before releasing information. They may require a copy of your drivers license. It would save you time to have all the required documentation done first before finding out after the fact. Especially, since corespondance is by mail.
