It really doesn't matter what you college major is, as long as you take all the required courses for entry into medical school. There are no particular courses in college that will lead to a particular speciality in medicine. All medical students take the same core courses, and core rotations. In your 4th year, you will have some elective rotations where you can further explore specialties you are thinking of pursing in residency (specialty training after medical school)

I usually recommend that people specifically NOT take a "pre-med" major. IF you don't get into medical school, or IF you decide not to go to medical school, you want a major where you can actually get a job. You can find jobs where a Biology major is needed, a Chemistry major is needed, a Business Major is needed, or even a Communications major is needed. NOBODY places ads looking for people who have "pre-med" as a major

To get into college you will need to take the SAT (some colleges accept the ACT exam as an option). The MCAT is typically taken in your Junior/Senior year of college. There are no other examinations until you get into medical school, then there are National Board Examinations - Part I, II, and III, and then after your residency program, you will have to take the Specialty Board exams.
