Abdominal Pain; Swellings

My apologies in advance, as this will be lengthy, even with me trying on to hit the highlights. A brief history: I have had unexplained fatigue for most of my life (I am 33). I have had heart irregularities such as faint & rapid pulses, skips and doubled beats, most of my life - always disregarded by Dr.s. I am prone to sinus infections, having had many over the years, including a very bad bout last Dec - this March. Ive had my bottom wisdom teeth cutting since I was 17, and have regular infections around the teeth/gums, along wtih tonsil/throat problems because of the infections. I had an impacted infection in March. I have had episodes of migraines: a 2 yr. period of them in my mid 20s and a constant, 3 week period earlier this year.

Family History - Cancer is #1, as it has killed everyone in our family, except for my brother, who had diabetes (but committed suicide). My father was 52 (esophogeal cancer). I had an aunt who was 53 and had had multiple cancers from in her 20s on (cervical, breast, and eventually bone). My maternal grandmother had leukemia ((have wondered if that could be a possiblity with me)) & grandfather throat cancer - from smoking. My paternal grandmother had breast cancer that spread; grandfather died from mouth/throad cancer - from smoking.

In May I woke up one morning and within an hour or 2 I had a noticeable mass in my upper, center abdomen that felt like a large fist. Over the next few weeks it spread out a bit and eventually became a sort-of elongated, football-like shape that is from the mid point between my navel and under the breasts. It is now angled out and down from the mid point of the upper abdomen.

Over the past few months I have had pains from the left side to the right. Initially the pain radiated from the center through to my back, then I had some moments/days of pain on the left area and side. Then it went back to the center area - the abdomen being tender/sore to touch. For the past couple of months, although the center section still aches/hurts and is tender & swollen, the more severe pain has been on in the right area, on my right side, and on around to the back. This is constant for the most part. ((Please note that an ER visit in May, after 71/2 hours, yielded a diagnosis of acute abdomen and I was told it was most likely just a gallstone in a duct. I had my gallbladder REMOVED many years ago, and was told I had no stones, it was diseased. A demanded ultrasound by my primary Dr. revealed no stones. That was all the Dr. wanted to mess with and I can not afford a $200 (min.) for a specialist)).

Other symptoms Ive had, and still experience: Extreme fatigue. Low-grade fevers that will come & go for a week or so, followed by a week or so with no fever. Chills; excessively cold hands and/or feet - comes and goes. Nausea. ((Have vomitted twice, once within the past month)). Headaches - come and go, not quite the same as migraines in intensity or localization. Getting winded upon exercise, lifting or much activity. Feeling as though I cant get a full breath, (this comes and goes, but has been more noticeable this past month). Appetite that comes and goes.

Other symptoms experienced, but not currently having: Itching - in the abdomen area. I had 2 (or 3) pencil-sized, reddish-bruised looking places appear on my abdomen about a 1 1/2 - 2 months ago, for no reason. Took about 2 weeks for them to go away.
Faintness/dizziness when standing... this only happened a few times, and not one time after the other.

RECENT: During the evening of 9/26 I was catching up on mail online and began to feel a tightening sensation in my legs and feet. Upon looking down, I was shocked to see that my calf muscles, ankles and feet were badly swollen (about double in size). Within minutes they hurt from the tautness. I also had a horizontal _line_ on the top of my right foot, about an inch down from the big toe, and in towards the center, that felt like a needle. The line was a light red color. I immediately got up and moved around, walked in place, stretched, etc. - to no avail.

I spent the next day (27th) resting as much as possible, as I was even more tired than usual. The swelling subsided some overnight, but was still noticeable throughout the day on the 27th. In the afternoon I experienced a feeling of being stuffed with cotton/ bloating all over my body (head, fingers, back, etc.). The sensation didnt last long, and has not returned. Later last night, during the evening hours, I had a multitude of moments of sharp, stabbing-type pain in a very specific area, on the inner edge of my right breast. They have not returned thus far. I was a bit worried with these pains because I have not had anything quite like them in the past... I had a (rightside, 2nd or 3rd) rib twist out of place years ago on a hiking trip and had subsequent relapses with it moving and ended up with arthritis-like symptoms in that area - these things continue to bother me on occasion, but I had not been having problems for sometime (months) prior to last night, and hadnt lifted or done anything to re-injure it.

I have noticed some unexplained weight gain. I rarely weigh ((I am considerably overweight, and have been all my life)), but know that Ive gained at least 4 pounds in the past month, despite not eating nearly as much, and eating much healthier the biggest part of the time.

Due to lack of money, no insurance, and the way I was dismissed & treated by the ER Dr & my primary Dr., I have not returned or sought a new Dr.. I have, however, done a lot of my own research online. Up until this past couple of days, my bet would have been pancreas problems, as it is the one condition that keeps coming up within the sites/information Ive searched. However, with the recent swelling, etc. Im wondering if it isnt a serious heart problem.

I dont smoke (never have, but was around it growing up). I dont drink. No drugs of any kind, except Verapamil (150 mg) that my Dr. gave to me months ago to try for my fatigue ((no tests have ever been run for a heart problem... I did wear a holter monitor years ago, for 24 hours, and the Dr. told me it was normal; - stating that the lowest was 50 some beats and that it often spiked to 140-150 and above... the highest being 181! ~
As for tests from the ER, they did a urinalysis, in which all the results were within the normal range, and they did some blood work. I dont know if this information will help, since it was done back on May 22-23, but I thought I would include it. (Im only listing the ones that were out of the range, and a couple on the end, one way or the other that I thought might be indicative of something.

Again, my apologies for the length of this. I thank you for your time and ANY help. ((Its looking like Im not going to be able to get any financial or medical assistance and will probably, eventually, end up at another ER... Should this happen, Id like to have an idea what could be wrong & what tests would be the best to do first - so that I can try to convince the Dr.s to help me)).

Would someone PLEASE help me with this. I have been waiting for 3 days now, and no one will reply. I didnt intend for the post to be so long, but didnt want to leave out potentially necessay information.

All I am asking is for someone to help give me a better idea as to what might be wrong (whether one or more possiblities), and the best test (s) to ask for in order to - hopefully - find the cause the quickest.

Hi Jennifer,

It looks like you have a lot of things going on. I understand that you don't have medical insurance, but without seeing a doctor in person for an exam, I'm not sure how much assistance we can provide you. My suggestion would be to get a fasting blood glucose test for diabetes due to the fact that it runs in your family and you are "considerably" overweight. Also ask the doctor about the possibility of latent autoimmune diabetes which is sometimes mistaken for type 2 diabetes in people who are in their thirties or older. The swelling in your legs while sitting at your desk, cold hands and feet, could be due to poor circulation, either from being overweight, diabetes, or heart problems. So, my next suggestion would be to see a doctor for a stress test. If they find no heart problems, then I suggest beginning a walking program to get your blood circulating and set the stage for losing some weight. I also suggest you get regular paps and breast exams due to your family history. And do breast self-exams every month, seven days after your period starts. Breasts have a tendency to be lumpy right before periods start, so you can get a feel for what your breasts normally feel like and have a basis for comparison. If possible, you should have your wisdom teeth removed because you shouldn't be having recurring infections due to this. The migraines could be caused from any number of things, and you've only had one since your twenties, several months ago. The diagnosis of acute abdomen is a pretty generic term for anything that upsets the tummy. With the other symptoms you mentioned, it could possibly be gastroenteritis, and maybe even a viral form which would explain the fevers, nausea, vomiting. The stabbing pain in your breasts could have several causes. This is called mastalgia, and it is generally causes by hormone fluctuations. There are other causes, such as benign cysts, but they are usually not anything serious.

I'm not sure how much assistance I have given you. And I know you were trying to avoid seeing a doctor due to lack of insurance, but I do urge you to find a doc who will check you out and let you make payments. If you don't feel I have helped you in any way, please do not accept my answer.

Lori, Thank you for taking the time to read my post, and for trying to help me with this. At the moment I am not concerned with pap or breast exams (although I do regular breast exams already =o) and my brother was the only diabetic in our family. The glucose in the urinalysis back in May was _normal_ and, although I have not one recently, I have had the glucose test in the past and was fine. I am fairly well aware of the signs of Type II diabetes, having lived with my brother when he developed it, and have not had any of the tell-tale signs ((excessive thirst, increased urine, craving sweets, etc.)). And the migraines - I apologize for not making it clearer that I had bouts with them throughout my mid 20s and then the really bad, long-lasting (2-3 wks straight) migraine earlier this year. ((Just wanted to clarify that, not that I think there is probably much of a connection between the headaches & stomach problems)).

I _have_ been to the doctors - to the ER because of the stomach swelling & pain and a follow up with my primary doctor who only wanted to check for gallstones, despite my not having a gallbladder. Once the ultrasound she insisted on came back ok, she didnt want to help me any further, and left me back to square 1. I cannot afford to keep paying her $50+ a pop, or pay $200 (to start!) for a specialist, or the $1500 or so to have my wisdom teeth removed. ((That said, I am trying to get financial help to get my health taken care of, but it, of course, is going to take time - and that is if I qualify.))

I dont believe the swelling is because of my being overweight alone - as I have been overweight all my life, and all these problems with my upper abdomen & the swelling in my legs, ankles and feet (which is now accompanied by a heavy feeling) has just came up, seemingly, out of nowhere. I honestly think there must be something more going on.((I do exercise every day or every other day, for at least 15-20 minutes, despite my very extreme fatigue)).

If you want to reply to me again, I am certainly open to any more information you may be able to offer. I cannot say that I feel the first post has helped me very much because I guess I am not understanding what kinds of causes for my abdomninal (and other) symptoms you are suggesting - aside from diabetes. ((Would the latent autoimmune diabetes or type 2 cause the pain and swelling in my abdomen? the back pain? the heart irregularities?)) Thank you again for your attempt to assist me. =o)

Dear Jennifer,

I'm sorry I could not be of more assistance. I really do believe you're going to have to see a doctor due to the magnitude of your symptoms. But I understand being in a financial position that makes that difficult. Please do continue to seek financial assistance to get these things looked into. I hope that you do get regular paps as you have a familial history of cervical and breast cancer, and cervical cancer is easy to treat when caught early. I still think you should have a stress test to see how your heart is doing. You don't say how overweight you are, but the swelling and back problems can be brought on by obesity, even if you've been about the same size/in proportion to your age, your whole life. You might also think about having your thyroid checked (I don't remember if you've already done that), especially if you've noticed dry skin and hair, because hypothyroidism can cause the fatigue and cold hands and feet you reported. One more thing I wanted to touch on here--I don't know the reasons your brother committed suicide, but if there was an issue of depression, please keep in mind that depression can run in families, and sometimes emotional pain can manifest itself in physical ailments. Please do not take offense to this, as I am only trying to look at different avenues, and I'm not in any way making any type of judgment about you or your physical pain that you are suffering. It's just something I felt I needed to mention. Again, I wish I could better help you, and I will definitely alert other experts on this site to see if anyone can be of further assistance. We have wonderful doctors and nurses here (of which I am neither), and they might better be able to assist you. I'm going to do that now, so hang on and hopefully you'll get some answers to your questions! Thank you for the opportunity to at least try, and for trusting us with your time and patience!

Dear Lori,

A big thank you - again - for replying, and for trying to get others to respond! I really, truly appreciate all your effort. =o)

To answer a couple of things in your last post : I have had occasional back problems most of my life, due to a curvature of my lower spine, but didnt connect that to the recent pain as, at first, it was radiatinged the middle of my abdomen, and is now off center to the right of my lower back and seemed to _move_ as the abdominal pain shifted more to the right.

I suspected thyroid problems way back in my teens, because I had always had an easy time gaining weight and a very difficult time losing it, no matter what I tried. This is still the case, but over the years I have been checked about 3 times and the tests always come back negative. I don't have dry skin or hair. (Not to say that things havent changed, chemical, in my body, as they obviously have). I just wonder if things like being overweight or thyroid problems could cause such severe, sudden symptoms like those of my abdomen. (When it started, in May, it was out of the blue and very, very painful - even though the pain and symptoms have now varied).

As for depression.. It does run, somewhat, in our family, (brother, cousin, ?), and Im sure it was an _issue_ with my brother as he had had a very difficult time accepting his diabetes. But, I believe his suicide was because he could not stand watching our dad die, and knowing he was going to pass. (My brother killed himself 3 days before dad died). I, too, have had my share of depression throughout the years, but have learned coping skills, turned to my faith, and believe I have learned how to cope well. Had I not been able to cope with losses last year as well as I did, I do believe that depression could be a cause. That said, at this point I am not going to rule anything out as a possibility - just looking to narrow the field, so to speak!
