I am 40ish and have no known health problems. I am currently experiencing a shooting pain that shoots up from my lower back up the spine. I've also been experiencing fatique and stiffness of neck. What could this be? No injuries or falls. What kind of medical care should I seek, especially since I'm self-paid?

It sounds like you might be suffering from a compressed nerved. I suggest seeing a chiropractor who can do Xrays if necessary, but who can also manipulate and adjust that area to relieve pressure and pain. They are generally pretty cost effective and, depending on exactly what is going on, can often alleviate most, if not all, of the pain in a visit or two. Sometimes it does take longer for maintenance should you choose to do that. And this happens often without any known injury or trauma--sometimes just from normal wear and tear on the body. In the meantime, try ibuprofen and ice (twenty minutes on, forty minutes off, several times throughout the day).
