why are pap smears painful is it physical or emotional

I was sexually abused at 15. I had to undergo a Rape Kit at that time. That was such a
tramatic and painful experience I have never had another Pap Smear because of the pain inserting the speculum. I am 40 should I be worried?I have no symptoms but I have been on HRT since age 15 due to a genetic condition....
