Question

undiagnosed recurrent r. knee pain, inflammation

Sent to Health Experts February 27, 2006 6:49 a.m.

my r. knee would become edematous, painful and would increase in response to increased activity, especially w/ climbing stairs. i had authroscopic knee surgery 6wks ago and immediately it got much worse. i had my knee drained x2 and was then admiited back to hospital for sepsis. none of my cultures grew any results but the first synovial fluid cx had extremely high wbc's present. now the knee has never been pain free, but now it is again inflammed, edematous and hot and very painful. noted vein distention and a purple color to r. knee and leg while up on leg. negative ana. wbc 8.8. plts. 478. sed rate 90 and crp 1.8. ana titer 1:160. dna neg. ena's <20. rfe <10. anti ccp <20. dated lab 1/30/06. then on 2/10/06 joint fluid neg for crystals.   while in the hospital i was treated w/ 3 wk course of iv vancomycin and po levaquin that i did not completely finish. strangely enough a coworker also had the same procedure done at the same out pt surgery center and he also got infected and his cultures grew enterococcus fasculitis and he was out for extended period.   my sister has lupus. i was told i was legally blind at age 40 but corrected w/ contacts but do not have an underlying etiology for my eye sight loss. i am extremely sensative to sunlight and burn very easy. i was diagnosed w/ gerd and inflammation of sac of intestine at age 38. i do suffer from anxiety related to public speaking and do have some episodes of depression, now controlled w/ effexor. i was told i was allergic to own sweat because i get a white or sometimes purple rash after working out at the gym, some spots on abd and back never go away. i used to run marathons and after when i would get rubbed, the masage therapist would say, did you run, your muscles aren't tight at all. i have lost 30 pounds from 200# to 172# but mostly during acute phase after surgery. urine screen was wnl. my blood pressure does sparactically increase at times but i have been unaware except in times when turned down to donate blood, etc. i do have a mild red flushed to forehead, cheeks and bridge of nose during heat or nervousness but it does return to normal in between times, no thickening of skin noted.   about 3 yrs ago i had inflammation of bilat elbows after being layed off of job and was diagnosed w/ tennis elbow by company md and was given steroid inj in one elbow and that elbow resolved in symptoms at 7wks and other elbow resolved at 6wks. i have seen rhematologist, orthopedic surgeon, infectious disease doctor and no one seems to know what to do for me. i am hesitant to ask for pain med as i feel they think i might be drug seeking. my girlfriend is rn and seems to be convinced that this is somehow some misdiagnosed autoimmune of some type. at present i did have stressful week at work and weather here in dallas was very cold and more rain we have seen all yr. and now knee is 3 times size of nonaffected l. knee. edema does extend down r. leg, no sign of clot.   what do i do. what type of md do i see next. what can i do about chronic problems, ie pain, and stiffness r. knee that is becoming contracted.   orthopedic surgeon states he had never seen joint/ knee quite like mine ever. he described ligaments as rocky road and very fraid and he had to smooth them down and does not seen knee like this one except in elderly.   please help. thanks, littleton. i am an engineer at airline manufacturer for job and have had an extremely emotional year w/ relationship changes, etc.

Personally I would see an internal medicine doctor to rule out any systemic disease such as lupus. Also ask your doctor about Prednisone. This is a steroid anti-inflammatory that might work wonders with your knee.
