I have been diagnosed with sever depression. I have been treated with EffexorXR for over two years. For the most part the treatment is working great in my life, the few side effects that I do have are well worth the stability I most often feel while on the medication. After being on EffexorXR for almost two years, I started to develop more severe signs of depression beginning with my menstral cycle which increasingly progressed each month into a severe bout of depression, nervousness, dizziness, lightheadedness, virtego and a sense of confussion along with elevated blood pressure.

My doctor, who isn't the one who started me on EffexorXR decided that I should increase my daily dosage. I went from 150mg to 225mg daily. That seemed to help for the first couple of months. Now, the menstral cycle depression is back and very strong and I am experiencing some of the other symptoms occasionally.

I am not confenced that this is a symptom of my depression as much as it is a hormonal issue that is triggering depression symptoms. Could you give me some feedback, information and/or recommendations? I am going to be finding a new doctor, one who is more interested in solving the problem rather than simply treating the symptoms. But for now, I would like to get a better understanding from a doctors point of view so I will better know how to discuss these problems with a doctor.

I am 36 years old, I have had 3 successful pregnancies, tubal after my third child (which is when the symptoms began to get severe 7.5 years ago) and am otherwise a healthy woman.

Prozac - 2 weeks of every month around my cycle. This seemed to help alot but it also left me not caring about anything and very emotionless. EffexorXR - started on 150mg daily for almost 2 years. Worked great in the beginning then during the last year my symptoms began and continually got worse around my cycle. EffexorXR 225mg - started about 4 months ago, very concerned with my elevated blood pressure. Helped symptoms over the first couple of months but I just spent a solid week of severe depression that has drastically affected my life. These symptoms literally start and stop over night in conjunction with my cycle.

I am sorry to learn that despite treatment, your depression is getting worse.

Since now you are having depression for over two years, you must realise that you need more than just anti-depressant to treat yourself.

Ideally, a three prong strategy should work in your case. First of all you must request your consultant to start you on CBT (Cognitive Behaviour Therapy). CBT is scientifically proven to help people with depression alone or better still along medicine.

Second Anti-depressant is known to loose effectiveness over period of time. And honestly two years is too much for medicine to remain effective without any reasonable break in between. So, you should try to take a break from Effexor XL for some time to come now. Here is evidence that changing anti-depressant or adding on a different anti-depressant helps in treating depression. Thirdly there are anti-depressant, whose trial have shown better result than Effexor XL and you should have tried anyone of the them by now. Remember in mental health disorders trying to get on most effective medicine is a matter of trial and error and it often takes some time to search the most appropriate medicine for oneself.

Try asking your consultant to take you off Effexor XL and try putting you on Escitalopram (brand name: Lexapro) an SSRI or on some other tricyclic ant-depressant or combination of both.

Last but not least try to get a bit of physical exercise. Exercise is again proven to beat depression especially drug resistant depression. Good luck.

Reply to Salcom's Post: So you dont think that this is probably some type of hormonal imbalance rather than a depression issue? Effexor works great except during my menstral cycles.

Also, how long do you think I will suffer from the side effects of getting off of Effexor, I am aware that I will have to decrease my dosage gradually. Can I take another type of anti-depressant during the time that I am decreasing my Effexor or do I have to get completely off of the Effexor before trying something new?

I am concerned about having to go without any medication for a period of time. I am a single mother of 3, full time worker and full time student in college. I dont have time nor the energy in my life to suffer from depression in an attempt to get better. I know that sounds silly but a little help from something else during my wening off period will definately make the difference between me being successful or slipping back into a very deep depression and possibly hospitalized which is where I was when I started on Effexor.

Yes, hormone imbalance is one of the reason, which can occur naturally and cause depression. But remember life tragic/tough incidents are other reasons, which can contribute to existing depression or could themselves become cause of depression, like death of someone dear and near.

Right now we know very little about psychiatry. As they very rightly say, psychatriy is still a soft science, as compared to other fields of medicine. For example, if someone breaks his/her leg, we can immediately take an x-ray and show here is fracture, so a patient has fractured leg.

On the other hand side, in psychiatry, it is still difficult to prove disorder by such hard facts/tests as x-ray, though a little development has taken place. DSM-IV onwards are still paper, pencil and interview based diagnosis. This is one of reason, why psychiatry is open to abuse by a SMALL number of practitioners, for what ever reason, racism, pure dislike, hidden agenda etc.

No denying that Effexor- XL may have benefited you more than others. Same happens with almost all people suffering from depression, that one medicine suit them better than others, because of our different chemistry, tolerance, age, weight etc.

Dear, as far suffering from side effects of Effexor-XL are concerned, let the consultant take care of it. But remember for two years you have successfully combated some of its side effects along with evil of depression (the disease of soul), many of which never stop with passage of time.

that changing anti-depressant or combining it with another one often beats the depression.

Normally you will have to get off Effexor-XL before starting another medication, but a consultant would know if he/she can start new anti-depressant along with tapering off Effexor-XL, they do it many a times, but only on consultant's instruction.

I am sure keeping the severity of your depression and history of hospitalisation, in view, no wise consultant, would keep you without medicine. I know its difficult to suffer from anything ill, when life has so many responsibilities of its own.
But at the same time your children should realise that you are not well, so they should not only take their own responsibilities more but should take your complete care as well. And I am sure they would like to see a happy, loving and caring mother like you around them and for that it is must for them to realise that at present you need their help and care more than other way round.

Last, I am sure a combination of therapies and new medicine as felt suitable by your consult would be able to treat your depression successfully.

I agree with the previous expert somewhat, However I think you are right on with your question about hormonal imbalances. It seems with your menstrual is when you notice differences in your moods. Thats because during this time your body is produced many hormones or sometimes not producing enough. I would go to your doc and suggest he/she test for a hormonal imbalance.
