Digestive/abdomenal chaos

I haven't had a bowel movement in over a week, have been having extremely small ones for several weeks prior. This has been happening periodically for around a year - and, I haven't even been hungry in this period of time. I have severe back pains, difficulty breathing at times, and cramping when I lay down with a "back up" of acids overnight. I had symptoms of toxemia at one point, and consulted my doctor - who never responded to my question. It's getting worse - my entire abdomen was inflamed from my breasts to my navel with infection earlier this week, and no matter whether I eat a little or a lot - nothing comes out. I'm also frightened because I already found a lump near the base of my vagina during this time that wasn't there before - dark brown and misshapen. I had had lymphoma (benign) before - but, this is nothing like the same thing. With it having been a year, I'm afraid something has developed in my body. I tried enemas at home with a purchased bag - but, the water/coffee/fluid goes up, but begins cramping and gets stopped, and doesn't come back down. What can I do? I have medicare so can't afford the deductible for a doctor right now - and, also, any surgical procedure which requires medications, or medications - are against my faith. Are there any alternatives and what might be wrong?
