Last week I started having a severe headache with stomach upset (no vomiting or diahrrea). I have previously been diagnosed with excess stomach acid that does not reflux. I do take Protonix and sometimes Pepcid on an as needed basis. None of these meds have alleviated the stomach upset. It actually feels like a dull ache as opposed to the nausea of stomach acid. I do still have the headache as well. It has now been a full week. Also when the symptoms began I had general body achiness and a sore throat. What could be the cause of these symptoms?

Hello,

If you didnot have diarrhea/vomiting,what was the indication that it was a stomach upset.Please reply soon.

I use the phrase "stomach upset" to describe the sensation more than anything. When I am experiencing excess stomach acid, my stomach feels more nauseous than anything. I wasn't sure if this recent experience was a flare up because none of my back-up measures were working, i.e. taking pepcid or prilosec in addition to my daily protonix. This experience I would almost describe as a dull ache instead of the nauseous feeling. This morning I did take prescription prilosec and it seems to have been somewhat successful. I am seeing a gastroentrorologist tomorrow. Last summer I had a barium test that showed acid that was not yet refluxing up. I don't ever experience heartburn,etc. I also have a small hiatal hernia. I was worried that my daily protonix was losing it's effectiveness and that I would have to find alternative treatment. As I have written, I am feeling relief with the prilosec. Do you feel it is safe to mix H2's with proton inhibitors? How much is too much of each?

Hello,

You have a relatively severe case of acid peptic disease and having hiatal hernia is worsening the condition.By itself small hiatus hernia does not cause symtoms but it contributes to reflux oesophagitis.Hence the severity of acidity and the related symptoms like the dull ache as well as headache.

Proton pump inhibitor (omeprazole and its group) is used to treat it.Protonix is pantoprazole and prilosec is omeprazole.The fact that you are being benefitted by these drugs indicate that you might have developed erosive esophagitis.

Barrett's oesophagus is another risk in such circumstances.This is a change in epithelium of oesophagus due to constant exposure of the esophageal epithelium to the gastric acid.It has the potential for malignant changes if constant acidic trauma continues.

Treatment in mild and moderate cases is usually managed by medicines.A very severe case may require sugery.

Advice includes lifestyle and dietary changes which I am sure you may have been briefed by your doctor.For your convenience,I am mentioning it once again.These are-

  • weight reduction
  • sleeping with head elevation by 4-6 inches
  • avoiding situations where abdominal pressure is raised
  • avoiding smoking
  • avoiding meals with large quantities of fluids
  • avoiding fatty food,coffee,alcohol,mint,orange juice and chocolate
  • avoiding certain medications like calcium channel blockers, anticholinergics and smooth muscle relaxants.

It is good that you have relief with prilosec.

Protonpump inhibitors are superior to H2 blockers though the two can be prescribed together.The dosage is decided by the doctor depending on the case and its severity.
