Pain releivers

I have had 3 major surgeries (hip replacement complicated by staph infection) in two years and have been on narcotic pain releivers throughout. My peak dosage was10 dilaudid 4mg per day. This was 3 weeks ago.
   I want to know what the proper formula is for "weaning" a patient from pain medication is. I went from my peak dose 3 weeks ago to 8 -10mg vicodin per day to 4 -10 mg vicodin a day. I have had increased pain in my prosthesis during physical thearapy, stomach cramps, cold sweats, and cannot sleep. Is this normal?
