Question

In March 2006 I started having swelling/pain in the glands ...

In March 2006 I started having swelling/pain in the glands the right side of my face. At that time, I went to see my family PCP and was told that means I am health. After a visit to my dentist he refered me to an oral surgeon who said it was my parodit (sp) gland and gave me anitbiotics ( 2 series). This did help some. I then went back to my PCP and saw the nurse pract. who Rx antibiotics (10 days of Levoquin(sp)and 1 serie of another antibiotic. Since this did not completly stop the pain and swelling, she refered me to ENT who suggested I take Muscinex. Although this has help, I continue to have pain and some swelling in this area, especially when I eat and sometimes when I drink. Please help. Joan Chewning

I am unclear what this means. Also, do you recal if the oral surgeon did a biopsy or any other testing on the involved glands?

Do you have any chronic health conditions or take any medications regularly?

Is the swelling constant?

Do you feel any hard lumps in the area?

Are you running a fever?

To answer your quests: About 3 days after the swelling/pain started I wnet to see one of the Dr's who work in the office with my PCP.

The oral surgeon did not do a biopsy, but the did an x-ray. Told me there was an infection but no stone.

I have no chronic illness except GERD which I take Priolec

There is not any constant swelling/pain. Only when I eat. The ENT said this was because I had thick salavia, to take mus. and massage the area when I eat.

I do not feel any hard lumps and I do have a fever off and on. The fever is not constant.

Since you are not responding 100% to the prescribed treatment, it may be time to look harder for an underlying cause. Chronic swelling and pain in the glands is often seen in certain autoimmune type disorders such as Chronic Fatigue and Sjögren's syndrome (chronic inflammation of the lachrymal and salivary glands sometimes seen with rheumatoid arthritis as well). For this reason I would consider a consult with a rheumatologist, or a qualified internest. The gland should probably be biopsied at this stage (since conventional treatments have failed), in addition to blood testing: antibody testing for example.

In the meantime, avoid sour foods (dramatic stimulation of salivation often aggravates pain), add a vaporizer or humidifier to the room you sleep in, if you smoke stop, drink 8-10 8 oz glasses of clear fluids daily, avoid caffeine, high sodium/processed foods, and simple sugars, do not take antihistamines (Mucinex may be continued), and practice excellent oral hygiene (including brushing, flossing, and a "dry mouth", alcohol free mouthwash like Biotene). Use Motrin or Ibuprofen for the pain, and apply ice to the swollen areas.
