What can cause a penis to swell? He had a blister and he picked at it until it burst but the tip of his penis is still swollen. It sort of looks like a mushroom now.

With an open blister, he is at risk for infection so it should be evaluated by his physician. Genital herpes can produce blisters like you describe. Herpes can remain dormant WITHOUT ANY symptoms for years before having an actual outbreak. If he is having an outbreak, you will definitely want to have this treated. There are antiviral medications available to TREAT...NOT CURE genital herpes. A trip to the doctor for confirmation of diagnosis is needed.
