I have not heard anything to warrant neurology referral just yet. You have esotropia and if prisms are as large as they can go, you need to let your ophthalmologist know this is the case and ask him if he would consider surgery. I think it is important to get the facts straight and then go from there. If you can determine the exact reason why he feels surgery probably wont help, that would be helpful. If there happens to be an acute nerve issues causing worsening of your diplopia, this would be like an eye moving less than before, then that is expected to improve in 3 months time and surgery would not be appropriate.
