I am 15 weeks into my pregnancy and I am constipated ...

I am 15 weeks into my pregnancy and I am constipated all the time. I drink plenty of water and try to eat the healthy foods. Everytime I have a bowel movement I bleed from back there. Afterwards, I feel pain when wiping myself. Any suggestion on what this may be or how I can fix this.
